Is there a way to code a form with fields to fill out, and submit to somewhere viewable by admin? Or like a PM?

Sounds like she wants something like a Report to Admin button or possibly a bug report button.

You could always add a button that opens the message system with your main admin as the "to" field. It's cheesy but it would work.
