Install: Index, step 3

Im new ad this so...

I really dont know what is or how to get my databasa user, password and the databasa name.

Where I can find them or create them?

I fixed it, thanks. But..

Now I logged out of my site and when I refreshed to log in all crashed, I guess that I have that bad luck... So Im trying to re-install and built it up all again but for some reason Im not stuck at the "site.com/install/index.php" step, it gives to me error
